The Buckinghamshire Immunisation Team has moved to on-line consent for all school immunisations.
Please find attached a letter detailing all the relevant links required to complete a consent form. Please ensure that you complete your child’s form as soon as possible, and at the latest by the date indicated.
You are required to complete a form to either consent OR decline the Flu immunisation – please choose the Year Group your child is currently in.
