The school nursing team will be in school on 30 September to administer free Flu vaccinations to students in Year 7-11.
The team uses on-line consent for all school immunisations. The letter below details the relevant links required to complete a consent form. Please ensure that you complete your child’s form as soon as possible, and at the latest one full school day before the vaccination date.
If your child is in Year 12 or Year 13, please refer to the attached clinical risk group list to confirm their eligibility for the vaccine. If they are eligible, please confirm their risk group on the consent form otherwise they may not receive the vaccine.
Years 7 to 11 are required to complete a form to either consent OR decline this immunisation. Please choose the Year Group your child is currently in.
